Identification of anti-HLA class II antibodies

Clinical information

Diagnostic Utility:

Identification of anti-HLA antibodies allows us to detect the presence of donor-specific anti-HLA antibodies (DSA) in:

  • Patients being considered for hematopoietic stem cell transplantation.
  • Patients being considered for solid organ transplantation.
  • Platelet refractoriness.

Identification of anti-HLA antibodies allows us to detect the presence of recipient-specific anti-HLA antibodies in:

  • Suspected Transfusion-Related Acute Lung Injury (TRALI).

Method:

Solid-phase assay (Luminex).

The serum to be analyzed is incubated with microspheres that present HLA antigens on their surface. A human anti-IgG antibody conjugated to phycoerythrin is added, allowing visualization of the binding of anti-HLA antibodies present in the serum using the LUMINEX device.

Reference Values:

In the identification, the specificities of the anti-HLA antibodies are determined. The results indicate the specificities of the detected antibodies and their mean fluorescence intensity (MFI), for example: anti-HLA-DRB1*03:01 antibody (MFI 10,000). It is considered positive when the MFI is greater than 750 and exceeds the cut-off value of the MFI/LRA ratio.

Diagnostic Algorithm:

Following the diagnostic algorithm, if the result of the anti-HLA antibody screening is positive (for class I and/or II), identification of anti-HLA antibodies (for class I and/or II) is performed to determine the specificities of these antibodies.
